**HIRING**

POSITION: Social Worker

LOCATION: Mausuton, WI

DURATION: 8 WEEKS WITH POSSIBLE EXTENSION.

SHIFT: Monday-Friday, 9am-5:30pm

GUARANTEED HOURS: 40 HOURS/WEEK

PAY RATE: $50-$56/HR

Responsibilities: 

• Assists with discharge planning primarily in Acute Care.
• Work with IDT to identify needs.
• Discuss/arranges options, services, and resources to help with areas of need.
• Assist with Health Care Power of Attorney documents and other Advanced Directives as able.
